This form collects attendance and date preferences for our iftar programs. Please fill in your information carefully. Submitted information will be used only for Ramadan event planning.

You may select all days you are available.

Example: If you select 20 days but indicated above that you can attend at most 10 iftars, you may be scheduled for up to 10 iftars from the 20 selected days; selecting 20 does not guarantee 20 attendances.
